As part of the NDIS, Support Coordination is included in your plan as a funded support, enabling you to make the most of your allocated resources. Your Southern Rivers Support Coordinator will help you effectively manage and utilise your NDIS funding and assist you in understanding your NDIS plan. We connect you with suitable service providers, and ensure that your supports are working together to meet your goals.
We offer two levels of Support Coordination:
Level 1 focuses on building your capacity to connect with providers and manage your supports independently
Level 2 offers more comprehensive assistance for those with complex needs
Your friendly Support Coordinator will work closely with you, tailoring their approach to help you maximise the benefits of your NDIS funding. Because we want to empower you to thrive in your community and get the most out of life.

Help with assessments that may be required to determine the nature and type of your funding
Negotiate services and prices, develop service agreements
