NDIS Community Nursing is a funded support specifically designed for people living with disability who require nursing care within their own home or shared accommodation. The goal of community nursing is to help maintain and improve your overall health and well-being, while enabling you to live as independently as possible.
Our community nursing support is delivered by registered and enrolled nurses who are experienced in addressing the unique health needs of people living with disability. Our nurses work closely with you to develop a tailored care plan that considers your specific health needs. You’ll receive the clinical support you need in the comfort and familiarity of your home.

Medication management and administration
Continence management and support
Monitoring and management of chronic health conditions
Post-hospitalisation care and support
Assistance with tube feeding and nutritional support
