An individualised support plan details your unique needs and goals and illustrates how funded supports can improve your quality of life. This plan may include assistance with daily living tasks, skill development, community engagement, home modifications and more.
Your NDIS funding is based on your individual needs and goals, which are assessed during your planning meeting with a Local Area Coordinator (LAC) or an Early Childhood Early Intervention (ECEI) Coordinator. We help you prepare for these meetings and can accompany you for additional support.
Yes! The NDIS allows for flexibility in case your needs or goals evolve over time. You can request a review of your plan at any stage, ensuring you receive the most suitable support possible.
Southern Rivers can assist you in preparing for this review process and advocating for any necessary adjustments to your plan.
If your application is denied, you have the right to appeal the decision within three months. Southern Rivers can provide guidance and support during the appeal process.
